Cancellations:

If you are unable to keep your appointment we respectfully request that cancel your scheduled appointment a minimum of 24 hours in advance. Your cooperation allows us to see another patient who may require prompt attention.  If three (3) appointments are missed without giving 24 hours notice further appointments will not be scheduled.

Adolescent Patients:

If your child is under the age of 18, a parent or legal guardian must accompany the child to their first appointment or he/she cannot be treated. For follow-up appointments a letter must be sent to the office giving Dr. Bauman permission to treat him/her with out the presence of their parent or guardian.

Prescription Refills:

Prescription refills are handled during regular office hours. Please have your pharmacy fax your refill request to (513) 984-8759. We will review your medical record before refills are authorized. Please allow 24 hours for your prescription to be refilled. Please note we are only able to refill prescriptions for current patients.
